Please fill out 2 copies of the Client questionaire and client contact info (under individual counseling forms), one for each partner. 

 

On the Counseling policy form, you will both need to initial where indicated

 

Fees:

My standard fee is 135.00 per therapeutic hour (50 minutes for adults, 45 minutes for children) I am not currently on insurance panels but I can give you a receipt that you can file with your insurance company to get reimbursed as an "out of network provider"
